Bloody Nose (Frequent) w/other symptoms

I (male, 22, 6’ 0”, 200lbs, taking sertraline 50mg daily, non-smoker) have never had a bloody nose in my life up until about 3 months ago. Since then, I get one about every week, and it only ever bleeds out of my right nostril. Coincidentally (or maybe not so much so…) the very top, center, right side of my head feels like it has a finger sized cylinder I can point to where it feels… off. Much like a bruise, I would say. Occasionally I lose my train of thought, feel intensely disoriented, have a hard time focusing, and have to take a moment to ground myself. I know this sounds bad, but it’s something I didn’t think was much of an issue. This being because my sleep hasn’t been great and I just started teaching, so I figured the long days and little sleep were causing this part of the problem. Should I go in to see a doctor? I figured the bloody nose was from it being dry out (winter), but I’ve been using a humidifier and Vaseline. I’m starting think that it might not be something that I should ignore. Note: Prior medical history is practically none. I got my wisdom teeth out a month and a half ago. I also had a cardiac event where my troponin was VERY high (200+) about two years ago.
