Struggling with ED at 27 (and I’ve had it for 10 years)

So yeah, I’ve had ED issues since I was a teenager. I used to masturbate a lot and watch tons of porn. Having sex with a new girl has always been an issue, and I’ve had a girlfriend for years now, but the issues keep coming back. I’ve been taking a small piece of tadalafil (1.5mg maybe?) and that’s enough to have sex for me. However, I’ve seen three urologists and they all have been useless. I had some tests done a year ago. My total test was 4.47 and free test was 12.50. I know this it’s not ideal for my age so I went to an endocrinologist who told me to start trt, which I didn’t because he didn’t even ask me to try anything else before going that way. I workout 4-5 times a week, I’ve improved my sleep, I eat well, so idk why my test would be that low. I generally feel tired and my libido is low, so I’m pretty sure my test is causing this. I’ve tried supplements and nothing seems to work. I remember that a few years ago, my libido was higher and I would algo have wet dreams, but no more. I don’t know what else to do! I feel that maybe my repeated ed issues have made an impact on my hormones somehow. I’ve tried kegels too. None seem to work, and regular kegels make it worse. I do get hard during the night, but it seems to be throughout night hours and rarely when I wake up.
