High ankle sprain?

2 weeks ago, I slipped on a set of icy steps(only 2). I hurt my ankle, not sure which way it rolled. Hurt like hell at first but no bruising a light swelling. Had xrays- no fractures or breaks. Able to walk on it and bear weight. Had MRI- showed a grade 3 AITFL sprain and bone bruise on inner ankle(no pain or external bruising here). Ortho did xray under stress too and said things appear to be stable. He is recommending PT and no surgery at this time as the area could heal. Only pain or tenderness is in the heel and resistance when inverting, I feel it on the lateral side of ankle. Do tears always require surgery or no? Very anxious of this. Thanks all.
